Please visit our billing page for complete information on billing contacts and links to securely pay your bill online. Tax ID is a tax identificaiton number used for tax purposes. You may pick up records at an office, email or fax as requested. You simply need to complete an authorization form and submit a copy of your photo ID. We can then send your report to wherever you request.
As a courtesy to patients, any requests for a CD of your medical images will be provided at no charge. If your referring physician specifically requests films, SimonMed will provide them at no charge. This depends on the type of exam. In many cases your referring provider will discuss the results.
You can access your Patient Authorization Form here. In our experience most procedures receive same-day authorization. This can take from 3 to 12 days. Login using your email ID and password. This should be one which you had set up at the time of enrollment. Once logged in, you will see your name on the top right corner.
