South Shore MyChart offers a number of useful features to help South Shore Medical Center patients skip the phone call and manage their care more efficiently. Patients can send a secure message to their care team, schedule appointments including same-day sick visits , view immunization forms, and complete pre-visit paperwork. In addition, patients with standing lab orders can schedule their lab draws online as well - no need to call!
Please fax completed forms to Proxy access allows a parent, guardian, or other designated individual to connect to the South Shore MyChart account of a family member or friend. If the adolescent has their own email address, they can provide it and will be given a code to access their own account.
This form requires a witness signature and must be signed onsite at South Shore Medical Center. Pediatric and Disabled Teen proxy access allows you to securely communicate on behalf of a pediatric patient who is under the age of 13 or 13 years if the child is disabled.
