Job Overview: The Certified Diabetes Care and Education Specialist CDCES delivers comprehensive and seamless services that bridge the gap and integrate clinical and self management aspects of diabetes and cardiometabolic care. Team member will collaborate and provide comprehensive, patient-centered care, and education conducive to behavior change and improved quality of life.
The CDCES supports and advocates for people affected by diabetes to help the patient achieve individual goals. Provides quality diabetes self-management education and medical nutrition therapy limited to Registered Dietitians in individual and group settings both in-person and virtually based on assessed needs.
Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes. Manages virtual classes including presentation of material, answering questions, and pre and post class communication and documentation. Effectively demonstrates ability to evaluate pertinent labs related to diabetes and associated comorbidities such as cardiometabolic, renal health, etc.
Routinely reviews lab results and targets with all patients, to ensure their understanding. Works collaboratively with patient and provider to establish targets and individualized goals. Demonstrates ability to utilize technology to assist patient with diabetes care and education, in individual and population health diabetes services.
