An APS program manager and clinical caseworkers screen referrals for appropriateness via telephone. They then conduct an in-home assessment within three working days to evaluate allegations of abuse, neglect, or exploitation and determine levels of functioning and risk.
They provide case management as appropriate to decrease risk of further abuse, neglect, or exploitation. Eligibility: Any caregiver who lives in Hudson County and cares for a person aged 60 or older. The Assistance for Caregivers program offers short-term individual counseling to help alleviate the emotional distress anger, frustration, guilt, isolation, and depression that many caregivers of elderly individuals experience. Eligibility: Any caregiver who lives in Union County and cares for a person aged 60 or older.
Staff provide on-site supportive services to individuals in several affordable housing facilities in the community so that they may live as independently as possible. Phone: This program offers a food shopping service for homebound older adult residents of Union County who are unable to shop for themselves and have no primary caregiver who can shop for them on a consistent basis.
The shopping is done on Wednesday and Thursday mornings. Inmates who are at high risk for HIV are given referrals to go to a prevention program after release. Clients who test positive are referred to a case manager for discharge planning. The 1 Information and Referral number improves knowledge and access of the public to community resources by linking clients and the parish communities to Catholic Charities, the Parish Access Centers and other community agencies.
This program provides information and referral services to older adults and their families, links to support services, and assistance regarding alternatives to institutionalization. New Day Community is a program committed to enhancing the quality of life for adults with disabilities in the Archdiocese of Newark. The program offers a wide range of wholesome recreational and socialization events on a bimonthly basis, including festivals, dances, plays, socials, field trips social awareness, ecology, empowerment weekends , and a specialized one-week summer vacation camp that offers an opportunity for peer interaction.
